Liability Waivers
Required documentation for all Trailblaze USA UTV rental and off-road adventures
Required for All Participants
All riders must sign a liability waiver for Trailblaze USA rental and a separate waiver for the park before participating in any off-road activity. These waivers are mandatory and cannot be waived.
Trailblaze USA Liability Waiver
What This Waiver Covers:
- •Release of liability for injury, property damage, or death
- •Acknowledgment of risks inherent in UTV riding and off-road activities
- •Certification of fitness to participate and no medical conditions that would prevent participation
- •Alcohol and drug certification
- •Age verification (18+ required)
- •Photo and video release for promotional use
Understanding the Activity
Off-road UTV riding is an exciting outdoor adventure. Like all recreational activities, it involves inherent risks. By participating, you acknowledge understanding these aspects of the activity:
- •Navigating varied terrain and natural landscape features
- •Operating a vehicle in outdoor conditions
- •Exposure to weather and environmental elements
- •The physical demands of outdoor recreation
- •Potential for minor to serious injury, as with any outdoor activity
In addition to the Trailblaze USA waiver, each park may require its own separate waiver or permit. Please contact the park directly or ask our staff about park-specific requirements:

Participants must be at least 18 years of age. If you are under 18, a parent or legal guardian must sign the parental consent section of the waiver.
Valid photo ID is required at check-in.
